ABOUT US
To be your patient centered medical home, all care at Livingston Community Health is coordinated with you and your family at the center of everything we do. Our care teams consist of nurses, medical assistants, referral specialists, care coordinators and providers all working to ensure you get the care that you need.
We are now offering Telehealth appointments for our patients. During this COVID-19 pandemic we want to keep our patients safe from possible exposure to the coronavirus that causes COVID-19, improve access to care, and to save you an unnecessary trip to the doctors office.
Livingston Community Health is a non-profit community-owned health center providing comprehensive primary and preventive health care services to all patients regardless of their ability to pay.
